IMO, raise more than min pre. Raise more on the flop. Raise a lot more on the turn. And w/o reads or stats meh on the river, probably wouldve folded to his reraise (as played, wouldve been a lot more committed if raises were bigger).

Case for you min raising make it 2.2-2.5x and he most likely folds out trash however, the problem comes on the turn, you should of bet bigger, 1700 into a 5220 pot is weak, i could understand betting thin for value if you had say a set or better and there was no draws but all you have is top pair top kicker, you bet 3-4K on the turn you kill the hand or force villain into a bad call, shouldn't of bet the river either, check calling and keeping it cheap is fine with TPTK.

Raise more pre flop. You're betting for value on flop and turn, while also needing to price him out of drawing for the flush. As it's played, I think the aggression he shows on the river is telling you his flush got there, I very much doubt he is sensing weakness in your small bet amounts and trying to make you fold.

Imo I think you left out some very important info, which is info on villian, like what type of player is he, his stats, with what range is he defending his bb, your table image etc.

Nonetheless one thing I notice is that you cbet on the flop with the same amount as your pf bet, which was 1200. Generally speaking, that is a sign of weakness so you are inviting villain even more to call, while you should have been defending your TPTK must more aggresive against that flush draw. Make it more expensive for him.

My little 2 cents :)

Your line seems inconsistent. You take little stabs at the pot pre-, OTF and OTT, then call it off with TPTK. If your hand is good at the end, it must have been good OTF and OTT. So why wouldn't you bet more on either of those streets, never mind on both?
