Texas Holdem Newbie Question

I'm hoping more experienced Hold'em players can clarify this.

When the table gets down to three players, call them A, B and C, and player A is the dealer, what happens if player B gets knocked out?

Is the game immediately "heads up" with player C becoming the dealer and having to post a small blind while player A posts a big blind?

Or is it dead deal for player B, such that player A deals for B, after which we're in a heads up situation.

If it's the latter, then player A would be posting a big blind twice in a row.

Everything is done according to protocol. No players have to pay the BB twice in a row if a player gets knocked out. The dealer button alternates in a heads-up game and don't forget to consider the ante factor.
