Detecting how someone plays a particular hand to narrow their range?

Whenever I read about putting someone on a range and narrowing that range as the hand plays out, one thing which always comes up is to spectate how they play a particular hand, such as top pair, middle pair, how they play their draws etc;

The one thing I don't understand though, is how do you do this? I have been playing around notetracker and set up custom notes. As i'm fairly new to the whole note taking thing I setup a custom note to tell me when an opponent bets the flop with middle pair, but then checks when a scare card comes on the turn etc; which is fine. But I notice that this is a fairly common thing, and people may well do this when playing top pair too, so how do you differentiate between whether someone is playing top pair or middle pair etc;?
