Have played about 200 hands with Villain here, he 3bets light, the kind of stuff he's 3bet me with are A5o, A9o, 44.

UTG ($10)
CO - Hero ($21.90)
BTN (19.14)
SB ($8.74)
BB ($10)

Hero is dealt AKo

Preflop ($0.15)
1 fold, Hero raises to $0.30, BTN raises to $1.50, 2 folds, Hero?

If we were 100bb deep here I would happily 4bet/call a shove, but since we are 200bb deep how does his change things? I don't really want to get 200bb's in their preflop with AK.
It's also worth mentioning that his 3bet is on the large side, usually here he 3bets to more like $1.

I hate flatting as well. Id 4 bet and re-evaluate if he comes over the top.

I would 4 bet to about 4 bucks, and call if he shoves over the top.

If he's has happy 3betting dominated Aces, like A,5/A,9 etc I would happy getting 4betting and getting 200BB all-in pre with A,K if needs be and have him crushed the vast majority of the time.
This is like the best case scenario, bar having like KK/AA in this spot to have him crushed.
As for his sizing, hard to say really, but good hands are rarely wanting to bet this large as they are wanting to induce calls/4bets etc so even more justified stacking for 200bb.

The absolute worse play here is flatting OOP agaisnt someone who is 3betting light.
4bet>Getting 200BB in pre>flatting OOP

If hero is correct in how villian is playing, then I certainly woulden't 4bet/re-evaluate fold to a 5bet shove or whatever, if that's what you mean by re-evaluate I'd be happy getting it in vs this sort of player. If he has it this time, then good on him, but more times than not you're massively +EV to get it in, even 200BB deep as a huge favorite.
Also, because it's like a CO/BTN battle, he might have hands like A,J+ which are willing to 5bet/stack as he might think you are playing back or whatever the reason.

I've flipped QQ and TT for 400 BBs live. It's fun.

4bet calling a shove seems pretty retarded if he's not a really bad fish

Urrgh that sizing almost makes me want to fold.
I'm flatting. I hate flatting AK OOP but we really don;'t have much of a choice in the matter as 4betting is just turning our hand in to a bluff and 6betting a 5bet at these stakes is surely just going to run in to KK+.

Being 3bet out of position deep is the most horrible thing anyone can do to you in poker. We have AK ffs and we're still in a shit spot! Unless the table is amazing a deep villain sitting on my left who like to 3bet light is a very good reason to move table.

As a side note when you get to the left of someone whos deep (and your deep as well ldo) 3bet the living shit out of them with position with some nice drawing hands.

Also given his sizing I doubt I'm calling off 3streets even when I hit unless this guys a complete nutjob. Be much more wary on Kxx than Axx as you have 4x the combos of hands that beat you.

Some adjustments for if you cant move table:

If he flats alot of 4bets 4bet/fold for value.
If he doesn't have a 4bet flatting range start 4bet bluffing Ax hands.
If he is capable of 5bet bluffing 4bet stack QQ+/AK (this will be pretty rare).
If he is prepared to stack QQ and AK for 200bb 4bet/stack
If your have no firm reads on any of the above just flat and think hard preflop about whether he is capable of having anything other than KK+ when you hit your hand. (does AQ really pot the turn etc etc).
