Can the Nash equilibrium improve my results?

I play a lot of home tournaments, and I often find myself confused about how to adapt to the short stacks in the later stages. I understand that push/fold heads-up poker is solved, but the situations I'm in are neither push/fold, nor exactly heads-up. Still, I feel I could very easily commit the charts to my subconscious memory.

The only problem is: I have no real idea what to do with those numbers. I guess I could play push/fold on the button when I'm in the final 2? But that seems sub-optimal, even if it's unexploitable.

What's the best way for a small home game tourney player to take advantage of the charts?
