Have Addiction Issues, Staying too Long and Tilting

I'm playing sessions sometimes close to 12 hours, I start to lose my game after around 5 hours.... but I can't help myself and stay until I lose the winnings i had made... I feel like with this problem I would not make it playing poker professionally.

you should set yourself some specific goal and then it will be easier to end the game without wasting the money won

You should really step away when you are not mentally 100%, even a small break can make a big difference in your psyche. As soon as you feel your gameplay is suffering step away from the tables to refresh yourself and if that doesn't work just try to stop playing for the day until you can give it your best effort.

Set Rules for yourself and stick to them

I have found that setting rules for myself and sticking to them allows me to not tilt as often, or recognize when my mind is not correctly in the game. The best tip when on tilt is play only premium starting hands.

Be careful with this, as something like this can quickly get out of hand.
I agree with Denver, try to set a specific goal. eg. if you win a fair amount of money, and you feel that you're not winning anything else for like half an hour, an hour, stop playing. Or you'll lose your winnings.

Also perhaps you should try to put a time limit on your pokergame if you have addiction problems and it affects you emotionally. If your average playing time now is 12 hours, try to go down bit by bit. Depending on how much you are willing to take off at a time, this can go fast or slow. I think everyone has their own time limit, but try to remember it as the following: if poker starts playing with you, stop. Tomorrow's another day :)
