Which cases should I run it twice in the river and the turn?
Should I run it twice when I have a draw?

Running it twice doesn't change your chances of winning.
Over the long run, you win in that situation as often as the odds dictate.

If it's a big pot and you don't want to deal with big swings, run it twice.
If you don't mind the swings, run it once.

Be aware that if you're playing micro limit games on Full Tilt, you're paying more in rake if you run it twice. IE, you lose more money in the long run.

It really is all about swings. At microstakes you should be able maintain a solid win rate with solid play so I wouldn't bother. Its all personal preference though.

All of you are saying "running it once or more times doesn't change the outcome in the long run"
That isn't true - sometimes it changes outcome a lot. For example you hold AA v QQ flop all on T56r in and decide to run it more than once. That's very good for you, because with every run your opponent should have exactly two outs when running once. If you run multiple times and lets say opponent binks two queens in first run, he now is drawing dead for all of the next runs instead of having 2 outs.
So if you are the one with a made hand and your opponent drawing to X outs, you should run it as many times as possible, but if you are drawing, you should run it only once.
Every run has the possibility to decrease drawing player's chances.

running it more than once

I use to run it more than once when ever someone asked - but realized that wasn't happening in reverse. Finally decided that running it once was the best move for me - I play in a weekly 1-2 no limit game with the same players. Yes a few were unhappy that I will not run it more than once but at lease I am consistent

I think that running a hand more than once if you already have a made hand can be less profitable. Considering like one of the posters above me mentioned pair vs pair. Well if they hit quads on the first run it can be assumed that you are probably good on the second run. However you still have to deal with the variables of a flush or a straight and thats a gamble some players arent willing to take because of the high chance of something like that happening. I will run it more than once if I have say a high drawing hand just to improve my odds but never if I am holding a pair because of the high risk that comes with it. Just my own personal opinion here.

If you think that you have an edge then running more times is better for you.

All that running it twice does is lower variance. Your odds don't change.

With that said, run it twice, maybe even more if possible. Nothing kills my soul like a suck-out.

My opinion

Running twice should be used when you are at the turn or flop and the person went all in and the hole cards you have should be calculated. How much have the hole cards won recently.

always run it twice, unless you have a large edge, then deal with the pain in either direction.

There are people who swear it doesnt make a difference but it does. If you are on a draw run it twice, all other times run it once.
