In a cash game, the house takes chips from each hand.
What is the formula? Sometimes they take $1
Other times they take $10 or more...

In a casino, it's usually along the lines of 10% of the pot up to a certain amount (usually $4-$6 for 1/2 NL). If the stakes are higher, sometimes they'll do a time rake where each player pays every hour or half hour.

$10 out of the pot at a 1/2 NL game seems extremely steep...

How about 10% with a $5 cap? Standard?

That is about right. I think the cap at the 1/2 at one of the near-by casinos is about that. I don't recall losing too much more than that. I think its often maxed at around 4BB +1$. My incidental proof of that is at the 2/5 game I play the max rake is $21.
